P450 isozyme (IID6): - Like other selective serotonin reuptake inhibitors, fluoxetine inhibits the specific hepatic cytochrome P450 isozyme (IID6) which is responsible for the metabolism of debrisoqui ne and sparteine. Although the clinical significance of this effect has not been established, inhibition of IID6 may lead to elevated plasma levels of co-administered drugs which are metabolized by th is isozyme. Drugs metabolized by cytochrome P450IID6 include the tricyclic antidepressants (e.g., nortriptyline, amitriptyline, imipramine, and desipramine), phenothiazine neuroleptics (e.g., perphena zine and thioridazine), and Type 1C antiarrhythmics (e.g., propafenone and flecainide).

Precautions
Pregnancy and Lactation: - Safe use of fluoxetine during pregnancy and lactation has not been established. Therefore, it should not be administered to women of childbearing potential or nursing mother s unless, in the opinion of the treating physician, the expected benefits to the patient markedly outweigh the possible hazards to the child or fetus. In 1 breast milk sample, the concentration of flu oxetine plus norfluoxetine was 70.4 ng/mL. The concentration in the mother's plasma was 295 ng/mL. No adverse effects on the infant were reported.

Neurologic:
Frequent: Headache, tremor, dizziness or lightheadedness, asthenia.

Infrequent: Abnormal gait, ataxia, akathisia, buccoglossal syndrome, hyperkinesia, hypertonia, incoordinat ion, neck rigidity, extrapyramidal syndrome, convulsions, photophobia, myoclonus, vertigo, migraine, tinnitus, hypesthesia, neuralgia, neuropathy, acute brain syndrome.

Rare: Dysarthria, dys tonia, torticollis, decreased reflexes, nystagmus, paralysis, paresthesia, carpal tunnel syndrome, stupor, coma, abnormal EEG, chronic brain syndrome, dyskinesia and other movement disorders (includin g worsening of preexisting conditions or appearance in patients with risk factors neuroleptic malignant syndrome-like events.

Possible food and drug interactions when taking this medication
Venlafaxine (e.g., Effexor)—Using these medicines with fluoxetine or within 5 weeks of stopping fluoxetine may increase the chance of developing a rare, but very serious, unwanted effect known as the serotonin syndrome. This syndrome may cause confusion, diarrhea, fever, poor coordination, restlessness, shivering, sweating, talking or acting with excitement you cannot control, trembling or shaking , or twitching. If you develop these symptoms contact your doctor as soon as possible. Taking tramadol with fluoxetine increases the chance of having convulsions (seizures). Also, taking tryptophan wi th fluoxetine may result in increased agitation or restlessness and intestinal or stomach problems
